INFORMATION SHEET FOR PARTICIPATION IN RESEARCH

You are being asked to participate in a research study conducted by Susan H. Weaver, PhD, RN, from the Ann May Center for Nursing at Hackensack Meridian Health. You are being asked to participate in this study because you are currently a clinical nurse, administrative supervisor or night shift team member. The purpose of this study is to investigate the fatigue, sleepiness, and sleep quality of clinical nurses and administrative supervisors and to explore the night shift team members' perception of their experience working night shift.

Please read the information below and ask questions about anything you do not understand before deciding whether or not to participate. Your participation in this research study is completely voluntary.

If you decide to participate, you will be asked to complete a survey. The survey will take about 20-25 minutes to complete. By completing the survey, you are agreeing to participate in this research study. At the end of the survey participants will be redirected to another survey link if interested in having an opportunity to win a $25 gift card. As a thank you for participating in this study, 45 participants who completed the survey will be randomly selected to receive a $25 gift card.

There are no other alternatives to the study other than not participating. Participation is voluntary and whether you participate or not will not affect your employment in any way. You have the right to decide not to participate and complete the surveys.

Your answers to the survey are anonymous and cannot be linked back to you in anyway. If you do not want to answer a question for any reason you are free to skip it. There is no more than minimal risk to you when participating in the study. There are no foreseeable risk since the survey is anonymous and there will be no way to link your responses to you. There are also no direct benefits other than contributing to the body of knowledge about the fatigue and sleep quality of clinical nurses and administrative supervisors and night shift team members' perception of their experience working night shift. The results from this research study, which will be reported only as grouped data, will be submitted for publication in nursing journals and for poster and podium presentations at professional nursing conferences.
